    2016-17 Spark Charter School Application for Enrollment

    Please complete an application for EACH new student you wish to enroll.  Current students need not reapply

    1. STUDENT INFORMATION

    NOTE: You must apply for the next progressive grade level (no retentions or accelerations--see Enrollment FAQ for more information).
    If you are unsure of whether you reside within SSD, you may check the District's website HERE

    All Applicants will be asked to submit the following proofs of residency upon final registration:

    a. Parent's CA driver's licence, consulate ID, or passport
    b. One of the following: Rental agreement, lease, monthly mortgage payment, or property tax bill showing name and address
    c. One of the following: PG&E, water, cable, or phone bill showing name and address (dated within 45 days)
    d. One of the following: Car registration, payroll document, tax document, or bank statement showing name and address
    Please be sure to submit a separate application for each student.

    2. RESPONSIBLE PARTY INFORMATION

    These are the parties who can act on the student's behalf.  
    The first party listed will receive all school correspondence, including enrollment offers, and will be the primary contact.

    3. PARENT AGREEMENT

    I/we desire to have our student(s) enrolled in Spark Charter School (“Spark”). We recognize that parent participation is an integral component of Spark’s philosophy, and is essential to the successful implementation of Spark’s educational program for the benefit of all of its students. We understand that Spark can only deliver on its educational promise to the extent that families agree to participate, and that Spark will work with ANY family that wishes to participate to ensure their ability to do so in a capacity that is manageable for them.  We further understand that by law, no public school (district or charter) can require parent participation.  Parent participation at Spark is not a requirement for application, and no child will be denied enrollment because parents have not participated.

    o ensure that the developmentally-based, small group learning environment of the Spark Charter School can be effectively implemented for the benefit of all of its students, we agree that we will participate in a regularly scheduled shift of two (2) hours per week per child (with a maximum of 6 hours per family).


    To ensure that Spark may provide an enriched educational experience to our child and all Spark students,
    we will serve on one school-wide committee in addition to our regularly scheduled classroom shift.


    To ensure that Spark may deliver a hands-on learning environment, which will include a minimum of five (5) field trips per classroom per year, we will make every effort to drive or chaperone on at least two (2) field trips per year for each of our enrolled children.

    To ensure that we are effective partners as classroom aides and/or recess supervisors, we will make every effort to attend the social-emotional intelligence training at Spark. This is a one-time only training.

    To ensure that we are effective partners in our child’s education at Spark, we will make every effort to attend the classroom volunteer training sessions, and any classroom and school-wide parent meetings. 

    To help create a campus that is conducive to learning, we will make every effort to participate in at least one Campus Work Day per year.

    To maintain a safe and healthy school environment, we understand that prior to participation, submission of a negative TB test is required of each on-campus volunteer.

    To ensure a safe school environment, we understand that, prior to participation, each on-campus volunteer must be fingerprinted and background-checked by an agency authorized by the Spark Board. We further understand that results of such background checks will, at the discretion of Spark's Executive Director, be a basis for determining the scope and terms of our participation.
